About Dr Duncan Couch

Hi, I’m Dr Duncan Couch, a UK GP who qualified as a doctor in July 2007. I help neurodivergent adults and health-curious people understand their bodies better, reduce overwhelm and improve energy, mood, gut health, focus and long-term wellbeing through small, realistic changes.

My approach is based on I-SNEMS: inflammation, sleep, nutrition, exercise, microbiome and stress. It combines medical knowledge, lived experience and a deep belief that better health does not have to come from extreme routines. Often, the biggest changes come from simple steps repeated often.

My story

I did not come to this work from a place of perfect health.

For many years, I lived with multiple chronic health problems including asthma, high blood pressure, underactive thyroid, low testosterone, chronic cluster headaches, migraines, neck pain from disc problems and multiple allergies. On paper, I was a doctor. In reality, I was also someone trying to function inside a body that often felt inflamed, exhausted, painful and unpredictable.

I also struggled deeply with my mental health. By 2020, I was severely depressed and addicted to opioid medication and alcohol. My life had become smaller, darker and more frightening than I ever imagined it could. I was not simply “stressed” or “burnt out.” I was unwell, overwhelmed and close to losing everything.

Going to rehab in 2020 changed the direction of my life.

Recovery did not happen because I suddenly became a different person. It happened because I started learning. I learned about addiction, psychology, nervous system regulation, trauma, habits, shame, neurodivergence and the way the body responds to stress. After rehab, I was diagnosed with ADHD, which helped me understand so much of my life through a completely different lens.

That diagnosis did not feel like an excuse. It felt like a map.

It helped me see that many of the things I had judged myself for were not moral failings. They were patterns, sensitivities and survival strategies that finally made sense. It also helped me understand why so much generic health advice fails people with busy, sensitive or beautifully complicated brains.

Since recovery, my life and health have changed dramatically. I have been sober since 2020, and the process of rebuilding my health sparked a genuine love of learning that has never really stopped. I became fascinated by inflammation, sleep, nutrition, exercise, the gut microbiome, stress physiology, neurodivergence and the practical things that help the body feel safer, stronger and more regulated.

The changes in my own health have been hugely positive. My migraines and cluster headaches stopped after rehab. My asthma and blood pressure improved. My neck pain settled. I no longer needed pain medication. Most importantly, I began to feel healthier, stronger, calmer and more alive than I had felt for years.

That does not mean I believe everyone can fix everything with lifestyle changes. That would be far too simplistic, and it would not be honest. Medicine matters. Diagnosis matters. Proper medical care matters. But I also know from both professional and personal experience that the foundations of health can change the way the body functions in powerful ways.

This is why I created I-SNEMS.

I-SNEMS stands for inflammation, sleep, nutrition, exercise, microbiome and stress. It is my way of helping people understand the body as a connected system rather than a collection of separate problems. Poor sleep can worsen mood and focus. Stress can affect digestion and inflammation. Gut health can influence the immune system. Exercise can support brain function. Nutrition can either calm or aggravate the body. These pieces are not separate. They speak to each other constantly.

My work is especially focused on neurodivergent adults because I know how easily health advice can become another source of shame. Many people with ADHD, autism or neurodivergent traits already know what they “should” be doing. The problem is that most advice is designed for an imaginary person with unlimited energy, perfect executive function and a quiet nervous system.

Real life is messier than that.

My aim is to make health feel more understandable, more compassionate and more possible. I do not want to give people another impossible routine to fail at. I want to help them find small changes that feel easy enough to start and meaningful enough to repeat. Because repeated small changes can become powerful over time.

If you are tired, overwhelmed, inflamed, foggy, stressed, inconsistent or frustrated with yourself, I want you to know that you are not broken. Your body may be asking for better support, better signals and a kinder way forward.

That is what my courses, guides and resources are designed to offer.

Simple education. Practical tools. No shame. No extremes. Just a clearer way to understand your brain, support your body and start feeling better from the inside out.
